Summary Statement: Improperly guarded floor opening. The danger tape does not provide adequate protection against a fall. The opening should be guarded – either covered or have railings/toeboards around it. Additionally, debris (grain) on the walking surface poses a slip/trip/fall hazard. Area should be kept clear of debris.
